Fatayat NU didirikan oleh tiga serangkai tokoh perempuan muda Nahdlatul Ulama pada 24 April 1950 di Surabaya,

.yaitu Nyai Hj. Khuzaimah Mansur (Gresik), Nyai Hj. Aminah Mansur (Sidoarjo), dan Nyai Hj. Murthosiyah (Surabaya).

Organisasi ini dibentuk untuk mewadahi pergerakan perempuan muda NU, dengan dukungan KH. Mochammad Dahlan.
